This has been based on collaboration between primary and\nsecondary care and through national partnerships.<\/p>\n\n\n\n<p>The new guidance promotes tailored and timely\ndischarge care for adults who have had a hospital admission complicated by AKI.\nIn particular, it highlights a need to address poor outcomes following AKI for\npeople with heart failure.<\/p>\n\n\n\n<p>The <a href=\"https:\/\/www.rcgp.org.uk\/clinical-and-research\/resources\/a-to-z-clinical-resources\/acute-kidney-injury.aspx\">guidance table<\/a> and the \u2018<a href=\"https:\/\/www.rcgp.org.uk\/clinical-and-research\/resources\/a-to-z-clinical-resources\/acute-kidney-injury.aspx\">Top Ten Tips\u2019<\/a> are designed to\nsupport safer transitions of care and are relevant to both hospital and general\npractice teams.<\/p>\n\n\n\n<p>The\nguidance was produced in partnership between the RCGP and <a>NIHR\nCollaboration for Leadership in Applied Health Research and Care Greater\nManchester; NIHR Greater Manchester Patient Safety Translational Research\nCentre; the Academic Health Science Network (AHSN) for the North East and North\nCumbria; Kent, Surrey and Sussex AHSN; Think Kidneys; NHS Education for\nScotland; and Healthcare Improvement Scotland.<\/a><\/p>\n\n\n\n<p>It\nhas been endorsed by the British Society for Heart Failure and the Primary Care\nCardiovascular Society.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>The RCGP has launched new national guidance for post-discharge care following acute kidney injury.
