MI uses the patient\u2019s own motivators and values to make achievable and healthy lifestyle changes including adherence to treatment.<\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone wp-image-3455\" src=\"http:\/\/www.thinkkidneys.nhs.uk\/ckd\/wp-content\/uploads\/sites\/4\/2017\/06\/motivational-interviewing-cropped-300x271.jpg\" alt=\"\" width=\"144\" height=\"130\" srcset=\"https:\/\/www.thinkkidneys.nhs.uk\/ckd\/wp-content\/uploads\/sites\/4\/2017\/06\/motivational-interviewing-cropped-300x271.jpg 300w, https:\/\/www.thinkkidneys.nhs.uk\/ckd\/wp-content\/uploads\/sites\/4\/2017\/06\/motivational-interviewing-cropped-93x84.jpg 93w, https:\/\/www.thinkkidneys.nhs.uk\/ckd\/wp-content\/uploads\/sites\/4\/2017\/06\/motivational-interviewing-cropped.jpg 411w\" sizes=\"auto, (max-width: 144px) 100vw, 144px\" \/><\/p>\n<p>Features of motivational interviewing (Rubak et al 2005)<\/p>\n<ul>\n<li>Motivational interviewing relies on identifying patients\u2019 own values and goals, and uses them as a basis to stimulate behaviour change<\/li>\n<li>Motivation to change will occur from patients skills, knowledge and ability, that is not imposed on them<\/li>\n<li>It is designed to elicit, clarify and resolve ambivalence<\/li>\n<li>Resistance and denial is a signal to roll with resistance and explore ambivalence<\/li>\n<li>Eliciting agreement and reinforcing patient\u2019s ability to carry out and succeed in achieving an agreed specific goal is essential for success<\/li>\n<li>The therapeutic relationship is a partnership that respects the patients thoughts and feelings<\/li>\n<li>It is both a set of techniques and counselling style<\/li>\n<\/ul>\n<p><strong>What are the benefits?<\/strong><\/p>\n<p><strong>For patients<\/strong><\/p>\n<p>MI can enhance patient self-efficacy &#8211; the degree to which a person feels successful or effective in managing their health or their life. Patients may often feel overwhelmed, hopeless or helpless, and consequently experience lower self-efficacy. MI can empower the patient and can instil hope, confidence and action.<\/p>\n<p>Motivational Interviewing helps patients to voice concerns and find solutions. Patients involved in MI (compared to treatment as usual) have found they are more likely to: enter, stay in, and complete their treatment, attend and participate in follow-up visits, and make important improvements in general health and wellbeing. MI can improve medication adherence and may lead to fewer subsequent hospitalisations.<\/p>\n<p><strong>For staff<\/strong><\/p>\n<p>MI is a useful skill that can be used to enable patients to explore their options and hear themselves discuss pros and cons without pressure.<\/p>\n<p>MI helps the healthcare professional to:<\/p>\n<ol>\n<li>Master core communication skills<\/li>\n<\/ol>\n<p><strong>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; OARS<\/strong> \u2013 ask <strong><u>O<\/u><\/strong>pen questions, use well timed <strong>A<\/strong>ffirmations, listen <strong><u>R<\/u><\/strong>eflectively, <strong><u>S<\/u><\/strong>ummarise<\/p>\n<p>2. Recognise \u2018change talk\u2019<\/p>\n<p><strong>DARN<\/strong> \u2013 the <strong><u>D<\/u><\/strong>esire, <strong><u>A<\/u><\/strong>bility, <strong><u>R<\/u><\/strong>eason, <strong><u>N<\/u><\/strong>eed -to change<\/p>\n<ol>\n<li>Avoid triggering resistance \u2013 \u201croll with resistance\u201d<\/li>\n<li>Improve empathy<\/li>\n<\/ol>\n<p>MI is a client-centred, empathic and yet directive interaction between patient and health professional designed to explore and reduce inherent ambivalence and resistance, and to encourage self-motivation for making positive change goals<\/p>\n<p>Motivational interviewing uses a guiding style to engage with patients, clarify their strengths and aspirations, evoke their own motivations for change, and promote autonomy of decision making<\/p>\n<p><strong>How can it be used?<\/strong><\/p>\n<ul>\n<li>MI can be delivered by a range of healthcare professionals and lay persons<\/li>\n<li>MI skills can be used for everyday face-to-face or telephone consultations<\/li>\n<li>MI empowers the patient, unlike well- intentioned but overly directive helping approaches that may reinforce patient passivity or dependence on case management<\/li>\n<li>MI skills can be readily learnt by health professionals and lay persons by attending a short 1 day or longer course depending on previous skills and knowledge<\/li>\n<li>Three elements are involved: practicing a guiding rather than directing style, developing strategies to elicit the patient\u2019s own motivation to change, and by refining listening skills and responding by encouraging change talk from the patient<\/li>\n<\/ul>\n<p><strong>Useful Links and Resources<\/strong><\/p>\n<p>Linda&#8217;s Story: Click <a href=\"https:\/\/www.thinkkidneys.nhs.uk\/ckd\/case-studies\/lindas-story-personal-reflections-nursing-career\/\">here <\/a>to read about Transplant Nurse Linda Boorer&#8217;s personal reflections on motivational interviewing.<\/p>\n<p>Rollnick et al : MI in practice&nbsp; BMJ 2010<\/p>\n<p><a href=\"http:\/\/www.bmj.com\/content\/340\/bmj.c1900\">www.bmj.com\/content\/340\/bmj.c1900<\/a><\/p>\n<p>Rubak S, Sandbaek A, Lauritzen T, Christensen B.
