We have now returned two batches of Your Health Surveys from our Outpatient Department\u2019s Low Clearance Clinics, however challenges lie in the resurveying of these patients as they do not return to the clinic at fixed intervals.\u00a0 As a solution, we have kept a register of patients and plan to resurvey them every 6 months.<\/em><\/p>\n

\u00a0<\/em>\u201cFor the first collection extra staff were put into clinic however this is probably unsustainable \u2013 the goal is to integrate it as a routine part of clinic activity.\u00a0 We are hoping to learn from the experiences of Newcastle, Kings, Derby and Sheffield from Cohort 1 who have experience of implementing the survey \u2013 I am particularly interested in Derby and Sheffield who have implemented the survey within their Outpatient Department clinics.<\/em><\/p>\n

\u00a0<\/em>I would like to use the Patient View facility to integrate PAM\/PROM into consultations, supported by having PDF\u2019s of our patient\u2019s results which are uploaded within the trust computer system under letters.\u00a0 This will mean they can be opened easily as part of an appointment.<\/em><\/p><\/blockquote>\n

We have been encouraging patients to sign up to Patient View, and to a text messaging service, Florence. We\u2019re interested to see whether this technology can improve a patient\u2019s activation level.<\/em><\/p>\n

We were pleased to win the best poster award<\/a> at our Trust Audit and Quality Improvement event recently. We think this shows that activation is an interesting idea with relevance to a wider audience.<\/em><\/p><\/blockquote>\n

We are planning <\/em>to use PAM\/PROM to change consultations, conversations and behaviours in our clinics.\u00a0 Our main priority at the moment is <\/em>making use of the PAM to change our interactions with our patients<\/em>. We are also looking forward to testing the re-surveying process in one of our satellite clinics in June.<\/em><\/p>\n

\u00a0<\/em>The main challenge has been delays in data returns and thinking about how best to use PDF\u2019s and PAM in conversations. We are hoping that our <\/em>workshop<\/em><\/a> this month will address these challenges.<\/em><\/p><\/blockquote>\n","protected":false},"excerpt":{"rendered":"
