Jonathon Hope, Co-Chair of the Transforming Participation in CKD Programme represented the TP-CKD programme last month at the King\u2019s Fund Conference themed \u201cApplying tools and measures to support personalised care\u201d.\u00a0 <\/strong><\/p>\n

As the event organisers, The King\u2019s fund, explained:<\/p>\n

Providing personalised, coordinated and empowering care is a key aspect of the new relationship with people and communities set out in the NHS Five Year Forward View.<\/em><\/p>\n

\u00a0<\/em>There are a variety of tools and measures available, such as the patient activation measure (PAM), to help assess the requirements of patients and populations and tailor care and support accordingly.<\/em><\/p>\n

\u00a0<\/em>The true value comes in how this information is used practically by provider organisations in the NHS, social care and the voluntary sector to ensure that they deliver the right care and support, and help people develop their skills, knowledge and confidence. This event focused on the practical application of patient activation measures and tools to support personalised care.<\/em><\/p><\/blockquote>\n

This is extremely relevant to the TP-CKD Programme, particularly in the current phase<\/a> (phase 2) of the programme where we are looking at how renal teams can respond to and improve patient activation levels.\u00a0 Jonathon Hope was able to share how the TP-CKD Programme has engaged clinicians and patients in the concept of activation.\u00a0 His presentation was entitled \u201cCase Study \u2013 engaging clinicians and patients in the concept and importance of activation\u201d<\/strong> and you can view his slides from the conference here<\/a>.<\/p>\n","protected":false},"excerpt":{"rendered":"
