In 2018, the NHS England personalised care team<\/a> and the UK Renal Registry<\/a> agreed to work together on a quality improvement initiative to follow up the work of the Transforming Participation in Chronic Kidney Disease (TP-CKD) programme.<\/a><\/p>\n

This work will focus on one of the interventions<\/a>, developed by TP-CKD, to test its effectiveness in practice and the difference its use makes to people living with chronic kidney disease.<\/p>\n

The new programme, TP2, will build on the success of the TP-CKD programme and will underpin the move beyond measurement towards changing practice and improving patient outcomes.<\/p>\n

\u201cIntroducing a coaching intervention<\/a> to clinical staff to enhance a patient\u2019s skills, knowledge and confidence (their activation<\/a>) will be a driver for delivering an improvement in clinical activation, patient activation and may also be associated with improved patient outcomes\u201d<\/em><\/p>\n
