The TP-CKD programme is about supporting and helping patients with kidney problems to develop the knowledge, skills and confidence they need to be involved in actively managing their own health care. This is known as patient activation. We are also interested in understanding how kidney problems affect the way people affected by CKD live their lives.<\/p>\n
The programme is rapidly gaining momentum and kidney patients from the 10\u00a0 Cohort 1 renal units have completed the \u2018Your Health Survey\u2019. The survey measures how involved people feel they are in their own health care, what symptoms they have and how they feel they are managing their day to day life.<\/p>\n
One important thing patients involved in the development of this programme have told us is that they\u2019d really like to see the results of the survey they participated in. Of course this is good practice but we know that this often this doesn\u2019t happen and as a consequence people can lose interest and a sense of involvement and connection with projects.<\/p>\n
The TP-CKD team is delighted that the results of the \u2018Your Health Survey\u2019 are available to view on Patient View screens, for patients who are registered to access it. We want to encourage patients who completed the surveys, and who are on PV, to access the screens and read their results. We\u2019ll be reminding patients that their results are available on PV with a poster in patient areas in the units taking part. Using PV to report results has great potential and can help patients lead and change conversations for a more person centred approach.<\/p>\n
If you have access go onto PV and see what is available and encourage as many people as possible to register. If you are unable to access PV but took part in the survey, ask your clinical teams for the results as they have been made available to them.<\/p>\n","protected":false},"excerpt":{"rendered":"
