The\nTransforming Participation in Chronic Kidney Disease (TP-CKD) programme is\nabout people with chronic kidney disease (CKD) working closely with their renal\nteam and others so that together they can learn how best to live life to the\nfull by developing their knowledge, skills and confidence. <\/p>\n\n\n\n

Since 2018, the Transforming Participation 2 <\/a>programme has developed partnerships with four renal units who are working to transform the way people are involved in their own care. A Peer Assist event on 8th<\/sup> February 2019 brought these units together, giving teams and patients an opportunity to share their learning, challenges and successes from the quality improvement programme, and to plan the next steps in their journey.<\/p>\n\n\n\n

The programme of the day can be viewed here.<\/a> \u00a0Discussions included the NHS England Universal Personalised Care Plan<\/a>, the feasibility of implementing health coaching techniques <\/a>into kidney consultations to support a patient\u2019s knowledge, skills and confidence, using the Patient Activation Measure<\/a> as a tool in tailoring care, as well as how Quality Improvement methodology can help teams to identify their next steps and future plans for change and improvement.<\/p>\n\n\n\n
