This event has been arranged for the Renal Units in Cohort 1a and 1b participating in the Transforming Participation in Chronic Kidney Disease Programme. This exciting and unique Programme aims to help patients living with kidney failure to feel better supported to reach the clinical and personal goals that are important to them.
A major part of the programme is to measure and understand how skilled, knowledgeable and confident patients feel about managing their health and well-being so that in partnership they can be better supported to build on their existing knowledge and understanding .
The aims of this Learning & Sharing event for the 10 Renal Units in Cohort 1a & 1b involved in the programme is to:
- Begin to create a culture of education and knowledge sharing
- Bring together clinicians from renal teams and patients in roughly equal numbers to form a network which will help design and develop the TP-CKD programme.
- Draw on the experiences and lessons from participants’ knowledge and experience as well as learning about barriers and challenges the programme may face
