SUMMARY:Transforming Participation in Chronic Kidney Disease Programme Learning and Sharing Event
DESCRIPTION:This event has been arranged for the Renal Units in Cohort 1a and 1b participating in the Transforming Participation in Chronic Kidney Disease Programme.  This exciting and unique Programme aims to help patients living with kidney failure to feel better supported to reach the clinical and personal goals that are important to them. \nA major part of the programme is to measure and understand how skilled\, knowledgeable and confident patients feel about managing their health and well-being so that in partnership they can be better supported to build on their existing knowledge and understanding . \nThe aims of this Learning & Sharing event for the 10 Renal Units in Cohort 1a & 1b involved in the programme is to: \n\nBegin to create a culture of education and knowledge sharing\nBring together clinicians from renal teams and patients in roughly equal numbers to form a network which will help design and develop the TP-CKD programme.\nDraw on the experiences and lessons from participants’ knowledge and experience as well as learning about barriers and challenges the programme may face\n\nProgramme Outline \nMorning session 10:00 – 13:00 \nWelcomes\, introductions and overview of the Programme and Workstreams. \nQ&A Session \nGroup Work – Learning Needs Analysis \nLunch 13:00 – 13:30 \nAfternoon session 13:30 – 16:30 \nGroup work – development of Renal Unit Plans \nNext Steps \nA more detailed programme for the day and joining instructions will be sent to everyone who registers\, along with information about travel\, claiming expenses and any pre work or reading that may be required before the event. \nTo register for the event please fill in the form below.
