We hope other regions will join this work over the next 18 months, to ensure more people are given the opportunity to undertake dialysis at home.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Our project<\/h2>\n\n\n\n<h3 class=\"wp-block-heading\">The DAYLife project driver diagram<\/h3>\n\n\n\n<p>Our project aim is to improve the quality and uptake of home dialysis.<\/p>\n\n\n\n<p><a href=\"https:\/\/improvement.nhs.uk\/resources\/driver-diagrams-tree-diagrams\/\">Read more about driver diagrams\nand how they are used in improvement projects on the NHS Improvement website\nhere.<\/a><strong><\/strong><\/p>\n\n\n\n<figure class=\"wp-block-image size-large\"><img loading=\"lazy\" decoding=\"async\" width=\"1225\" height=\"885\" src=\"https:\/\/www.thinkkidneys.nhs.uk\/kquip\/wp-content\/uploads\/sites\/5\/2020\/09\/Driver_final2.png\" alt=\"\" class=\"wp-image-8162\"\/><\/figure>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Who is the project run by, and who is it for?<\/strong><\/h3>\n\n\n\n<p>The DAYLife project is run by KQuIP, in partnership with Kidney Care UK, Baxter and NxStage.&nbsp; <\/p>\n\n\n<div class=\"wp-block-image\">\n<figure class=\"aligncenter size-large\"><img loading=\"lazy\" decoding=\"async\" width=\"360\" height=\"100\" src=\"https:\/\/www.thinkkidneys.nhs.uk\/kquip\/wp-content\/uploads\/sites\/5\/2020\/09\/DAYLife-logos.png\" alt=\"\" class=\"wp-image-8164\"\/><\/figure><\/div>\n\n\n<p>Our partnership is underpinned by a patient focus, and all project\nwork is carried out for the benefit of patients.&nbsp; <\/p>\n\n\n\n<p>The project supports multi-professional teams to learn and use\nquality improvement skills and leadership in order to work towards our shared\naim of improving the quality and uptake of home dialysis. <\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Why DAYLife?<\/h3>\n\n\n\n<p>At the moment there is wide variation in home dialysis rates across the country, unexplained by the demographic distribution of the local population. At present, only a handful of renal centres have more than 30% of patients dialysing at home. In 2017, the UK Renal Registry reported that 4.5% of dialysis patients undertook home haemodialysis and 12.5% peritoneal dialysis. <\/p>\n\n\n\n<p>Despite national professional guidance and patient groups stating more people wished to dialyse at home there has been little consistent progress in the last 10 years. For example, evidence shows that home dialysis can lead to:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Significantly improved quality of life for the patient \u2013 less\ntime and resources spent in or travelling to and from hospital, and fewer lost\nworking days<\/li>\n\n\n\n<li>Improved medical outcomes, for example reduced hospitalisation\nrates, improved recovery times, improved overall survival rates and reduced\nincidence of depression<\/li>\n\n\n\n<li>Improved knowledge, skills and confidence for the person living\nwith and managing their long term condition<\/li>\n<\/ul>\n\n\n\n<p>These\nbenefits are endorsed by NICE, and supported through the&nbsp;<a href=\"https:\/\/www.kidney.org.uk\/Handlers\/Download.ashx?IDMF=ffca233b-27c8-4bbd-ae3d-1d3cad97cd6c\">Home\nDialysis Manifesto&nbsp;<\/a>and the&nbsp;<a href=\"file:\/\/\/C:\\Users\\nbm0503\\Downloads\\Kidney_Health_progress_report_2017%20(12).pdf\">Kidney\nHealth: Delivering Excellence report<\/a>.<\/p>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p>We want to develop leadership and a culture in renal units which makes home dialysis the first option considered for dialysis.<\/p>\n<\/blockquote>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>How does the project work?<\/strong><\/h2>\n\n\n\n<p>The project is run <a href=\"https:\/\/www.thinkkidneys.nhs.uk\/kquip\/the-kquip-regional-project-pathway-and-events\/\">using the KQuIP framework<\/a>, incorporating quality\nimprovement and leadership training alongside regional collaboration and project\nimplementation.<\/p>\n\n\n\n<p>The DAYlife project is implemented using a three-step approach<\/p>\n\n\n\n<h4 class=\"wp-block-heading\"> Step 1: Research and discovery <\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Exploring what is working well on home therapy (HT) usage by centre and by region and why <\/li>\n\n\n\n<li>Analysing whether the same solutions can be applicable to other centres or regions <\/li>\n\n\n\n<li>Comparing contexts and looking at what process and outcome measurements can be used to monitor whether changes are resulting in the desired improvement<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\">Step 2: Ideas <\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Creating a list of possible solutions and their predicted outcomes<\/li>\n\n\n\n<li>Presenting and addressing these at a regional event<\/li>\n\n\n\n<li>Compiling a short-list of interventions\/solutions\/ideas that will be tested in the centres or regions<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\"> Step 3: Testing  <\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Step 2 should have created a menu of ideas for centres to agree to take away and try<\/li>\n\n\n\n<li>The project group will then plan the testing of these solutions\/ ideas monitoring their success using agreed metrics.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>How do we measure improvement?<\/strong><\/h2>\n\n\n\n<p>Clinical colleagues are often more familiar with measurement\nfor <strong><em>research<\/em><\/strong>\non a large scale with a fixed hypothesis. Health service managers and those in\nmore strategic roles may be more familiar with measurement for <strong><em>judgement<\/em><\/strong>\nas a way of understanding a level of performance. <\/p>\n\n\n\n<p>Measuring for <strong>improvement<\/strong>\nis different. <\/p>\n\n\n\n<p>The concept of measuring for improvement means that there\nneeds to be willingness to frequently change the interventions (as you learn\nmore with each test) and an acceptance of \u2018just enough\u2019 data, working with data\nand information that is \u2018good enough\u2019 rather than perfect. Measurement for\nimprovement does not seek to prove or disprove whether clinical interventions work\n\u2013 it seeks to answer the question \u201chow do we make it work here?\u201d.<\/p>\n\n\n\n<p><a class=\"wp-block-heading\">DAYLife measures<\/h4>\n\n\n\n<p>Quality improvement teams are encouraged to collect measurement for improvement depending on the change ideas being tested locally.&nbsp; Measurement covers <a href=\"https:\/\/improvement.nhs.uk\/resources\/measuring-quality-care\/\">outcome measures, process measures, balancing measures<\/a> as well as patient reported outcomes.<\/p>\n\n\n\n<p>Participating kidney units submit a simple set of core\nmeasures monthly to the national project partnership:<\/p>\n\n\n<div class=\"wp-block-image\">\n<figure
