Message to Cohort 2

You should all now be in the process of collecting your first batch of PAM/PROM surveys and CS-PAM collection – thank you!  Please do keep in touch to let us know how this is going, any challenges you are having or any success stories that you would like to share.  An extended end date of 10th April 2017 has been set for your CS-PAM collection, measuring how much clinicians at your units support patient activation. Thank you to all those clinicians who have already sent back their survey either electronically (click here to complete yours now) or on the paper versions available from the project lead at your unit.
