    Hello there,

    I wonder if I can get some clarification regarding whether a patient with the following renal profile is to be classified as having suffered AKI. I see quite a number of patients with this kind of profile and keep getting confused whether these should be labelled as AKI as we do not still have an e-alert system in place.

    This is a 63-year old diabetic lady who was admitted for joint replacement. Going by the definition of AKI as 1.5 times the reference value (the lowest value within 3 months of the event) this patient has suffered AKI but if you consider the 24-48 hour change, she has not! I would be very grateful for any clarification as this affects whether the patient gets told that they have had this complication and that they are at risk of CKD in the long term.
